You’re allowed 5 initial visits without additional paperwork. At your next visit, you’ll complete a brief progress report, and we’ll use it to submit a re-evaluation report (MNR) requesting authorization for further visits.
Your acupuncture benefits are part of your health insurance plan. ASH (American Specialty Health) is the company your insurance uses to manage authorization and claims for these benefits — ASH is not your insurance company. Here is how your visits, approvals, and re-evaluations work.

In practice, “unlimited” usually means up to 2 visits per month without a re-evaluation report. If more frequent or additional treatment is medically necessary, we submit a re-evaluation report (MNR) requesting further authorization.
Frequency limits may also apply. Some plans limit how often you can be treated (for example, up to twice per month) regardless of your annual visit count. Once we verify your specific plan, we’ll let you know exactly how many visits you have and how often you can be seen.
Applies to Kaiser “00” prefix patients, and to some “unlimited”-visit plans once the standard allowance is used.
At this visit, you’ll fill out a short progress report describing how your treatment is going.
We use your report to submit a re-evaluation (MNR) requesting authorization for additional visits.
You’ll be approved for a specific number of additional visits and a time window in which to complete them.
Please note
Your re-evaluation visit is focused on reviewing your progress and preparing this report. Treatment may not be provided on this specific visit.
Visits are approved within a specific time window — for example, 6 visits to be used within 6 weeks. If you’re unable to complete treatment within that window, due to illness, scheduling conflicts, or other circumstances, we can request a one-time extension.
Extension requests are not guaranteed to be approved, especially if the requested extension is long, so we recommend completing your approved visits within the original time window whenever possible.
Please ask us — we’re happy to check your specific plan details before you begin treatment, so you know exactly what your plan covers.
